Illinois State law requires that students entering the ninth grade or entering an Illinois school for the first time submit evidence of a complete physical examination and immunizations as required. Students entering the 9th grade who do not meet these requirements will be excluded from school attendance on October 15th. Please note the following health requirements and information. The physical form is due at the school no later than August 1, 2012. For safety considerations, students may not participate in physical education classes until the health examination form is on file at the school. Students will receive academic penalty for nonparticipation. Transfer students are given a 30 day grace period with which to comply with physical, immunization, and vision exam requirements. Students not in compliance will be excluded from school after 30 days. The Physical Examination:

The health of your child is important to his/her progress in school. Please contact the school nurse if your student has any special needs or requires medication during school hours.
