NURSE'S OFFICE - Health Requirements

Illinois State law requires that students entering the ninth grade or entering an Illinois school for the first time submit evidence of a complete physical examination and immunizations as required. Students entering the 9th grade who do not meet these requirements will be excluded from school attendance on October 15th. Please note the following health requirements and information.

The physical form is due at the school no later than August 1, 2012. For safety considerations, students may not participate in physical education classes until the health examination form is on file at the school. Students will receive academic penalty for nonparticipation.

Transfer students are given a 30 day grace period with which to comply with physical, immunization, and vision exam requirements. Students not in compliance will be excluded from school after 30 days.

The Physical Examination:

  • Only the current “Certificate of Child Health Examination” form will be accepted. The IHSA sport physical form does not meet the requirements for the 9 th grade physical.
  • Must be dated less than one year prior to the start of school.
  • The Health History portion on the back of the form must be completed and signed by the parent or the form will not be acceptable.

The Required Immunizations:

  • Diptheria, Pertussis and Tetanus : A minimum of 3 doses with the last dose after the 4 th birthday. Students entering grades seven through 12 who have not already received Tdap are required to receive 1 Tdap dose regardless of the interval since the last DTaP, DT or Td dose.
  • Polio: A minimum of 3 doses, with the last one being given after the 4 th birthday.
  • Measles, Mumps and Rubella: 2 doses, with the first being given after the 1 st birthday.
  • Hepatitis B : 3 doses.
  • Varicella: 1 dose on or after 1 st birthday or verification of disease history by health care provider.

The Visual Examination:

  • Students entering Illinois schools for the first time (home school, out-of-state or country transfer) must submit evidence of a vision exam completed within one year prior to the date of entering the Illinois School System. Illinois Eye Examination Report form only and must be submitted by October 15th.

The Sport Physical:

  • Students wishing to compete in athletics must present annually a completed physical examination form before they can participate. The 9 th grade “Certificate of Child Health Examination” form is acceptable.

The Student Health and Emergency Form:

  • Mandatory for all students.

The health of your child is important to his/her progress in school. Please contact the school nurse if your student has any special needs or requires medication during school hours.

Keep a copy of all health records for your files. All forms can be downloaded from the St. Francis website.
